If, at the time of your consultation, you decide you would like to take an appointment with the surgeon for your surgery, we will give you the operative consent form for you to take home and read carefully. If you prefer to think things through, we will simply send you the consent form by mail if and when you schedule a preoperative appointment with the surgeon.
